Birth Certificates

Congratulations on your new baby! In order to make receiving your baby’s birth certificate and enrollment in DEERS within 90 days of birth, please be aware of the following steps: 

  1. Complete the birth application form (after the baby is born). This will only be completed with the Birth Clerk. After, completion the Birth Clerk will input the information into the California system.

    1. If California department of public health (CDPH) denied the application due to errors, the birth clerk will make the correction and sign the form as birth clerk in certificate as a clerk, not parents or sponsor. 

    2. Parents have 5 days after the child is born the complete this form, if not completed on the delivery day.

    3. Please contact the Birth Clerk at 760-383-5293 if you have any concerns. 

  2. Complete the birth certificate request form given by the Birth Clerk. This form must be completed and notarized by the parents at the notary/ legal office.

    1. The legal office is located at bldg. 230 and their hours are Monday thru Friday 0800-1600 Thursdays 0800-1200, and their phone number is 760-380-5321

  3. Mail the birth certificate request form and include $32.00 money order or personal check payable to the San Bernardino Country. Their address is Vital Statistics Section, 340 N Mountain View Ave, San Bernardino, CA 92415 

  4. Birth certificate can be requested online via https://q-r.to/sbhospital. Compete online form and pay associated fees. Await confirmation that birth certificate has shipped.  

  5. Parents can email vs-birth@dph.sbcounty.gov to check the status of birth certificate and/or status of their order. 

    1. CDPH will send the birth certificate at least 2-3 weeks but at the earliest of 1-2 weeks. 

  6. Enrolling newborns under DEERS with certified copy of the birth certificate (within 90 days from date of birth) 

    1. Please contact DEERs at 760-380-2348 

    2. Failure to enroll will result in the child’s ineligibility of Tri-care benefits and may incur financial responsibilities of care at the medical facility. 

  7. After the newborn is enrolled into DEERs, contact Tri-care to get newborn enrolled under the sponsor.
