Primary Care

Weed Army Community Hospital uses the Patient Centered Medical Home (PCMH) model. PCMH is a team-based model, led by a physician, which provides continuous, accessible, family-centered, comprehensive, compassionate and culturally sensitive healthcare in order to achieve the best outcomes. The model is based on the concept that the best healthcare has a strong primary care foundation with quality and resource efficiency incentives.

The PCMH is a departure from previous traditional healthcare models because it focuses on the "whole person" concept, preventive care and early intervention and management of health problems rather than on high-volume, episodic, over-specialized and inefficient care. A PCMH practice is responsible for all of a patient's healthcare needs and for coordinating/integrating specialty healthcare and other professional services.

Services

Primary care also includes medically necessary diagnostic laboratory and X-ray procedures and tests. Primary care is used to treat adults and children, including infants and newborns. Common services include:

• Routine care, like an annual physical exam
• Medical treatment for non-emergency or non-urgent care
• Follow-up care if you have ongoing medical problems
• Preventive health care, like immunizations and allergy shots
• Family planning
• Patient education and counseling

Primary Care services include Ambulatory Services, Allergy/Immunology Service and Telehealth Services.

Family Medicine

School/Sports Physicals

Did you know that scheduling your child's annual or sports physical during their birthday month can simplify your planning? Physicals are valid for one year, covering school, sports, and CYS services. To streamline your visit, download and pre-complete the required form below before your appointment. Please Note:

  • If your child wears glasses, please make sure to bring them.
  • If you are coming from an outside healthcare facility or have recently arrived at Fort Irwin, please bring a copy of your child’s immunization records.
  • Children requiring an asthma action plan, school medication forms, or with other health concerns should schedule a regular appointment to have their additional needs addressed.

Immunization Clinic

Immunizations at WACH are provided on a walk-in basis during posted hours of operation. Walk-in immunizations are for children 4 years and older. Children less than 4 years of age can utilize the walk in clinic if instructed by PCM. Please bring in shot records when bringing children for immunizations at any age. After immunizations, patients must wait in the clinic for 20 minutes, to monitor for any adverse reactions, so please take this into consideration when walking in. Improving your patient experience: JOES, ICE and the WACH customer survey card.

Suggestions or improvements on how we may better care for you and improve patient services are welcomed by the command.

We invite you to fill out a WACH survey card, found throughout our facilities, after you use our services. The Interactive Customer Evaluation (ICE) is a web-based tool that collects feedback on services provided by various organizations throughout DoD, including WACH.

JOES, the Joint Outpatient Experience Survey, is a new DoD-wide survey used across all services to gather feedback on specific patient experiences during visits with their provider. When you receive a JOES, please fill it out within one month and give us your honest feedback.

Completing the survey gives you the opportunity to have your voice heard by WACH leadership. We use the survey results to identify areas for performance improvement. You can also formally recognize people or services that performed exceptionally. Additional funds for your hospital are also generated based on your responses and overall satisfaction ratings.

We take these surveys seriously and use your comments and suggestions in our effort to improve services, procedures and facilities to serve the community better.
